Use this to gather what your CPA will ask for before you file your 2025 personal return. If something doesn’t apply, skip it.

- Sale of stocks, bonds, or other investments
- Sale of other assets or property
- Installment sale details
- Cancellation of debt documentation
- Real estate transactions (1099-S and closing statements)
- Home purchase, sale, or refinance documents
- Primary residence sale documentation
Retirement and HSA
- IRA and retirement distributions (1099-R)
- Retirement contributions (IRA, Roth, SEP, SIMPLE, Solo 401(k))
- Rollovers (dates and amounts)
- HSA contributions and distributions
- 1099-SA (HSA distributions)
Extra Notes (If Applicable)
- Qualified charitable distributions (QCDs) from IRA
- Disaster-related retirement distributions
Education and Student Loans
- 1098-T (tuition statements)
- 1098-E (student loan interest)
- Out-of-pocket education expenses
- 529 plan distributions
- Student loan forgiveness documentation
Child and Dependent Care
- Child/dependent care expenses
- Care provider name, address, and tax ID
- Total cost by provider and by child
- Proof for claiming dependents
Health Insurance
- 1095-A (Marketplace insurance)
- Medical and dental out-of-pocket costs (if itemizing)
- Long-term care insurance premiums
Digital Assets (Crypto)
- 1099-DA (digital asset proceeds)
- Records of receiving, selling, exchanging, gifting, or disposing of digital assets
Deductions and Payments
- Mortgage interest (1098) and property taxes
- State and local taxes paid
- Charitable contributions (cash and noncash)
- Investment interest
- Gambling losses (if applicable)
- Major purchases affecting sales/use tax
- Tax preparation fees (if relevant)
Home, Vehicles, and Other Situations
- Employee stock benefit activity (Form 3922, vesting schedules, etc.)
- Clean vehicle purchase documentation and VIN
- 1098-VLI (vehicle loan interest), if issued
- Household employee wages and withholding details
- Foreign accounts or assets exceeding $10,000
- Gifts over $19,000 to any one person
- Disaster loss documentation (if applicable)
If you’re not sure whether something matters, include it anyway. Your CPA can ignore it, but they can’t guess it.
